You may qualify if any of these are true

You’re a Medi-Cal member
You have a chronic or high-risk condition
Common qualifying conditions include:
- Diabetes, heart disease, stroke, lung disease, HIV, cancer
- Pregnancy-related, nutrition-sensitive, mental or behavioral health conditions
You’re enrolled in a participating health plan.

Eligibility is determined by your health plan and may vary by county and condition.
