Fill out a short form: your Medi-Cal plan, your Medi-Cal ID, and your health condition. Takes about 2 minutes. Your answers are confidential and will not affect your plan benefits or services.

2. We confirm you qualify

We review your answers. If you qualify, one of our community health workers reaches out with the next steps.

3. Groceries arrive at your door

You get healthy groceries delivered to your home for up to 12 weeks, at no cost to you.

Who qualifies
You may qualify if all of these are true:
  • You’re a Medi-Cal member
  • You have a chronic or high-risk condition
  • Common qualifying conditions include:
    • Diabetes, heart disease, stroke, lung disease, HIV, cancer
    • Pregnancy-related, nutrition-sensitive, mental or behavioral health conditions
  • You’re enrolled in a participating health plan.

Who is Umoja Food For Health?

Umoja Food For Health works with participating Medi-Cal health plans in California to deliver healthy groceries to members who qualify, at no cost to them. We’re a minority-owned company, and because the groceries come through your own health plan, this is a real benefit your plan covers.

That’s right. If you qualify, there’s no cost to you, no copay and no bill. It’s covered by your Medi-Cal plan. Checking only takes about 2 minutes.

No. It’s an extra, optional benefit on top of your regular Medi-Cal, so it won’t replace or reduce anything you already have, and you can stop anytime. Your answers stay confidential, too.

Yes. We keep your information private and secure. We only use it to confirm you qualify, set up your groceries, and coordinate your care, and we don’t sell it. You can read our privacy policy for the details.

According to California’s Medi-Cal program, using Medi-Cal isn’t counted in a public charge decision (as of 2026). These rules can change and depend on your situation, so if you have questions about your case, talk to a qualified immigration attorney or an accredited representative. We can’t give immigration advice.
